So to make things a bit interesting, for my next adventure with my group, I was thinking of having them assist with a plague outbreak on the Shetland Islands (a stopover on their way to Islandia). Part of the reason is to hammer home how Fimbulwinter is affecting people - and living in close quarters, with too little of not very nutritious food, is prime breeding ground for the spread of disease.
I figured the plague (will figure out which one later, or make one up) will impose the Degeneration Condition +1 with an increase at a timed interval (an hour? A day? Six hours?), with proper care, food, etc slowing the rate of Degeneration until the person can heal on their own. Transmission being contact & in the same enclosed space.
I think spells like Apples of Idun can heal already accrued damage, but not remove the disease. I don't want the Dwellers to just magic away everything - they might be able to heal/cure one person at a time, but not fast enough to eradicate it without work.
Any way, these are just some of my thoughts. If any of you have any ideas on how to run a plague plot, I'm very interested! I'd like this to not be a 'magical' plague, but an example of the very real fall out of an extended dark winter.
