If I maintain a shrink spell, does that mean they maintain the -1 to size (so go from a 4 to a 3 until countered or the rune chain ends, for example)? Or does it mean each turn they lose 1 size?

While thematically fitting and a nice fantasy trope to further reduce the size more and more -- it technically "just" maintains the -1 size.

Can't check the actual page it's written on, but compare the logic to other maintains spells, e.g. a stance : you don't get more growing bonuses each round, just maintain their effect so you don't have to recast them. It's the same for the shrink spell.

But dont forget that focus can be used in the casting of this spell. So if I got Focus +2, i can shrink and maintain a size 4 (human) size to size 1 or like the spell says if I got Focus +3 I could make a frog of that size 4 creature by making it size 0 !!!
